Create an Adaptive Form template
An Adaptive Form template is specialized AEM Page Template, with certain properties and content structure that is used to create Adaptive Form. The template has preconfigured layouts, styles, and basic initial content structure. Every adaptive form is based on a template.

Before we create our adaptive form template, we need to create a folder which will hold those templates. So to do that, I’ve logged into my AEM instance, clicked on the Tools menu, and in the general, click onto the Configuration Browser. So here, you can either create a template in the global folder, or you can create a folder that will hold your template. So let’s create our first folder here, and we’re going to call it Eligibility Screener. And since this template is going to be editable template, we’re going to select the editable template option here and click on Create.
So now we have created a folder here called Eligibility Screener. The next thing we need to do is to create a template in this particular folder. So I’ll go back to my Experience Manager, go into the Tools here and click on the Templates.
So here is the folder that was created. I’m going to drill down into it and create my first template here. So I’m going to select Adaptive Form as the basis of my template, and give a template title here. So it’s going to be called Eligibility Screener, and click on Create.
So the template is created. I’m going to open it in the edit mode.
So every template will have initial content, structure, and the layout. So let’s take a look at the structure layer. So the structure layer determines what kind of a structure your template will contain. So typically it will contain a structure for the header, and this is the structure for the body of your form, and this is the structure for the footer. So let’s go ahead and add a component to this section here, and we’ll add the header component. We’ll add the header component, and similar manner we’ll add a footer component to our structure here.
We’ll add a footer. So now that’s our structure of our template here. So every form that is based on this template will inherit this structure. So there’s one more thing you can do. So you can either lock or unlock the components here. So when you unlock, that means the form author who’s creating the form based on this particular template will be able to modify this header component. So let’s do that here. And then the footer component, we’re going to leave it as locked, and we’re not going to add anything in the allowed component section or the form container. And in the initial content, we’re not adding any initial content into this form here. So since we unlocked the header component that was moved to the initial content layer, and we’re not changing anything in the layout layer as well. So now that our template is created, the next thing we need to do is to enable this template so that it becomes available to the form authors. So let’s go ahead and enable that template here. So it’s in a draft mode and click on enable to enable the eligibility screener template. So now any form author who is authoring a form and based on this particular template will see the structure that we had defined in this particular template.
